WAIVER RELEASE & INDEMNIFICATION

In consideration of the acceptance of my membership or participation in events with Strike Mixed Martial Arts. 1/5 Sunlight Drive Port Kennedy 6172

I waive all current and future claims that I may have against Strike MMA, its coaches, directors, members, volunteers, employees,
agents and sponsors, or its executors, administrators, heirs, successors or assigns (the organisers), and release them from all 
claims for death, injury or damage arising out of my participation in their club, their event and its related activities, together 
with any costs, including legal fees.
I agree to indemnify and keep indemnified the organisers against damage arising out of my participation in the events and its 
related activities.
I agree that I am solely responsible for any damages I cause to equipment or facilities and that I agree indemnify Strike MMA Club 
against any claims for said damage.
I agree to comply with all the rules, regulations and instructions of the organisers.
In the event of injury or illness during participation, I consent to receive medical treatment, 
which may be deemed advisable by the organisers or by the attending first aider and any subsequent medical professionals. 
In the event of accident, injury or illness during participation, I consent to receive medical assistance and treatment 
including blood transfusions and anaesthetic, and I agree to pay all such fees and expenses incurred. I understand that this 
does not in any way affect my ability to make a claim against any insurance that I may have.
I acknowledge that I have sole responsibility for my personal possessions and equipment during all events and activities.
I agree that Strike MMA may use  my name and picture in any broadcasts, telecasts and the press including 
website as they pertain to the Strike MMA and their events and activities. 
I may request Strike MMA to remove any image of me or use of my name by notifying a committee member in writing and that I will give 
Strike MMA a reasonable amount of time to make the amendment.
I understand that my membership or entry fee is non-transferable and not refundable.
I agree to have my personal details recorded and used by the Strike MMA for further communications of future 
related events. Strike MMA may pass, with my consent, my information onto third parties who require the 
information for the provision of goods or services that I acquire through membership of Strike MMA.

TERMS AND CONDITIONS OF ONLINE SIGN UP/MEMBERSHIP AND DIRECT DEBITS

This online direct debit sign up will be debited via SQUARE online Direct Debit Service through the website https://www.strikemmawa.com, for the time period/membership selected.
10 Class passes are a one time payment and valid for 3 months from time of purchase.
Unlimited memberships are monthly debits and will be an ongoing payment until member/client cancels by going into their sign in personal member account and canceling any further payments.
No lock in contracts.
Or alternitavely Emailing Strike MMA Administration for a cancelation or by physically visiting the Strike MMA reception and filling out a cancelation form.
Fees will be incured for each Direct Debit by Strike MMA's Square account with Strike MMA being charged a fee of 2.2% applied to payments processed through Square’s Transactions API.

Prior to comencement of training you will be asked to fill out a hard copy of liabilty waiver and terms and conditions.
If you are a minor (under 18 years of age) you must be accompanied by a parent or guardian for their written permission and signature of all required documents.
All online Membership Plans must be of 18 years or older or completed on behalf of youth member by parent or guardian.
